2012-2013 Registration Schedules

The following documents are required for student registration:
•    Birth Certificate
•    Social Security Card
•    Immunization Records
•    Grades (most recent report card)
•    Photo Identification Card or Drivers License of Parent/Guardian
•    Proof of Address/Residency (rent receipt, utility bill, etc.)

Note to Parents:
* Chaperones will be provided.
* There is NO FEE to attend Fish Camp.
* No student will be released to go home until Fish Camp is over.
* All students are expected to stay for the entire orientation.
* Please fill out the permission slip and transport your child to and from camp.
